Connector Improvement: Healthie FormHistory endpoint

An important concept in healthcare, and especially in the age of telemedicine, is understanding response rates to clinical questionnaires in order to quantify things like the potential impact of non-response bias.
In Healthie, information about questionnaires sent to individuals is captured in the FormHistory endpoint, which is not currently one of the endpoints exported by Fivetran. Adding this data would allow our clinicians to better understand patient follow-up and the evaluate the quality of patient care.
