Other: Add "apply - no deletes" (-md) to refresh and compare to 6.1

Since upgrading to HVR 6.1 we are unable to refresh or compare with "apply - no deletes" (-md).  This works fine in HVR 5.8 and we would like to have this added to v6.1 onwards.

    Indeed we failed to include this capability in version 6.1. We plan to re-introduce this in the coming months.
